Transitional Housing Placement-Plus-Foster Care (THP+FC) was established under Assembly Bill 12 (AB 12) to extend foster care beyond age 18 for eligible foster youth, up to the age of 21. This program offers various foster placement options, including Supervised Independent Living Settings, providing transitional housing and supportive services through a personalized Transitional Independent Living Plan. AB 12 was authorized by the Federal Public Law (P.L.) 110-351, the Fostering Connections to Success and Increasing Adoptions Act of 2008.
At EMBRACING LIFE’S CHANGES, we understand that their transitional period can be a daunting experience for youth as they often lack stable housing, financial resources, and the necessary life skills to navigate the complexities of adulthood. This is why our transitional housing program strives to maximize housing benefits for the youth we serve and for society as a whole. Besides access to stable and supportive housing, youth can focus on education and personal growth, and receive case management counseling and life skills training. They are also provided access to health and well-being services, such as healthcare, counseling, and resources for overall wellness.
